The honest answer
So — do you actually need a fixed retainer?
Let’s be straight about it: a fixed retainer is not compulsory. After Invisalign or braces everyone needs retention of some kind, but a removable retainer, worn as advised, can hold your result perfectly well on its own. A fixed wire is not about needing more retention — it is about needing less effort and less risk.
Its value shows up where teeth are most likely to creep back — closed gaps, rotated teeth, and above all the lower front teeth. Those lower incisors crowd most of all with age, and the evidence is consistent: teeth keep drifting for life even in people who never had braces (Sinclair & Little, 1983; a 50-year follow-up, Persson et al., 2022), and after treatment crowding carried on increasing for 10 to 20 years, with only about 10% of cases still well aligned (Little et al., 1988). For anyone who would honestly rather not depend on remembering a removable retainer, night after night, for the rest of their life, a bonded wire works whether or not you think about it. So the real question is less “do I need one?” and more “how certain do I want to be that my result lasts?”
What the research shows
Do fixed retainers really keep teeth straighter?
First, the honest caveat. The highest-level evidence — the Cochrane review of orthodontic retention — is clear that retention is essential, but finds no single retainer proven better than another with high certainty. The trials so far are relatively small, and more good research is needed. So no honest dentist can promise you a fixed retainer is guaranteed to beat a removable one for everyone.
That said, the evidence and everyday experience point the same way for the long term. In a five-year randomised controlled trial, a bonded retainer held the lower front teeth significantly better than a removable one. Just as telling, 72% of patients had stopped — or only rarely wore — their removable retainer by the five-year mark. That fall-off in wear is the single biggest reason removable-only retention slips over time, a limitation echoed across the wider systematic-review evidence.
There is a very human pattern behind that figure. Most people are diligent with a clear retainer at first — but after a year or two the motivation quietly fades, the odd night gets missed, and teeth need only a little freedom to begin drifting. By the time it is noticed, the movement can be enough to need a fresh round of aligners to put right — time and money spent again on a smile you had already paid for. A fixed wire worn alongside your clear retainers heads this off almost entirely.

Lower front teeth
The lower incisors are the most likely of all teeth to relapse, and the ones the evidence most consistently shows a bonded wire holds best. A discreet lower wire is the classic case for fixed retention.
Closed gaps & rotations
Spaces that have been closed and teeth that were rotated carry more “memory” and pull back hardest. A constant hold is the surest way to keep them exactly where they finished.
If routines slip
If you know a nightly retainer is the sort of thing that quietly falls off your radar, a fixed wire takes the decision out of your hands — it simply works while you get on with life.

Even diligent wearers value the reassurance of two layers: the wire holding the front teeth around the clock, and the clear retainers holding the whole arch and covering the wire if it ever loosens.

Living with a fixed retainer
A bonded wire is genuinely low-maintenance — it just asks for a slightly different cleaning routine and the odd check.
Clean around the wire daily
Floss cannot pop straight between teeth joined by a wire, so use a floss threader or a small interdental brush to clean beneath it once a day.
Keep up hygiene visits
Let your hygienist scale gently around the wire at your regular appointments — that is all it takes to keep the area healthy.
Check it now and then

Do you really need a fixed retainer, or is a removable one enough?

Can everyone have a fixed retainer fitted?

How long does a fixed retainer last?
On average around five to ten years before it needs maintenance or replacing, though many last longer. A bonded wire has no fixed expiry, but it can occasionally come loose at one end. It is worth a gentle check with your tongue now and then: if it ever feels loose or simply does not feel right, contact us to have it checked, because a broken fixed retainer can let teeth drift silently out of line without you noticing. Your hygienist also keeps an eye on it at your visits, and your clear retainers keep holding things while it is re-bonded.
Do fixed retainers damage your teeth or gums?
No — a bonded retainer does not harm healthy teeth. Nothing is drilled; the thin wire is simply bonded to the surface behind the teeth. It does ask for a little extra cleaning care: you floss around it with a threader or an interdental brush and keep up your hygiene visits, so plaque cannot build up along the wire. Looked after, it is a safe, low-maintenance way to hold your result.
Will a fixed retainer feel strange or affect my speech?
A well-placed fixed retainer should not affect your bite or your speech at all. It does feel a little strange to the tongue for the first week or two — that is completely normal — but almost everyone stops noticing it after that and settles in fully. The key word is well-placed: a poorly positioned wire can nudge the bite, interfere with speech or keep coming loose, which is exactly why it is worth having fitted properly and checked.
